PRS Cemetery Clean-Up (May 2023)

MT. CARMEL CEMETERY CLEAN-UP DAY MAY 7, 2023,
BUS DEPARTS BETH SHALOM AT 9:15 AM AND RETURNS AT 12:00 PM



 Students in Grades 5-10, will be joining Congregation Beth Shalom for the Mt. Carmel Cemetery Clean-up Day. While at the cemetery, they will also participate in age-appropriate activities that introduce Jewish Cemetery traditions, etiquette, prayers, and customs.

*Please complete the lower section of this form and return it to Civia White before MAY 1.

By signing my name below, my child(ren) has permission to participate in Mt. Carmel Clean-up Day through Congregation Beth Shalom. I hereby authorize the Director of Education or a person designated by the Education Director, to obtain emergency medical care for my child(ren) in the event such care is indicated. I give my permission for my child(ren) to receive emergency medical care by any nurse, doctor, paramedic or member of a medical staff of a hospital licensed by the state of Kansas or Missouri. I understand that every effort will be made to notify a parent/guardian prior to treatment.

I certify that my child(ren) is(are) in good physical health. They have my permission to participate in all activities that are part of this program.

I release Congregation Beth Shalom and its agents from any liability. The possession or use of alcohol, drugs, and/or tobacco products as well as behavior that endangers the safety of my child/ren or others, will result in my child being sent home from the program. If an offense does occur, I accept responsibility for my child/ren's actions and support the staff's discipline decisions.
